Site work and paving for a mixed-use development in Lawrenceburg, Indiana. Completed plans call for site work for a playground / park / athletic field; for paving for a playground / park / athletic field; sidewalk / parking lot; and sidewalk / parking lot.
This project is being bid on unit cost. The scope of work shall include clearing & grubbing, grading and construction of a new six space parking lot and approach drive. The contractor is responsible for traffic control during this process. The contractor will be responsible for any and all necessary restoration upon completion of this project. The City will stop taking questions Bids shall include the completed Lawrenceburg Bid / Quote Form, Form 96 w/ Financial Statement, detailed project schedule, Conflict of Interest / Nepotism affidavit and the E-Verify forms provided. The last two items are required by the State of Indiana for all Public Works Projects, effective July 1, 2012. All bidders shall be pre-qualified with the City's Safety Program, forty-eight (48) hours prior to bid opening. The contractor receiving the award shall furnish an approved Performance Bond and a Payment Bond, along with written proof of insurance with the City listed as an additional insured before receiving the Notice to Proceed. Project schedule shall be coordinated and approved by the City Director of Project Management and City Safety Coordinator. Contractor access to job site is over an easement through an active apartment parking lot. Coordinate with property owner to minimize disruptions in access by residents. Contractor is responsible for any and all traffic control per INDOT and OSHA standards during this process. Contractor shall be responsible for construction layout for the improvements indicated in the plans. City will pay for initial staking. Contractor will supply one Handicap parking sign (R7-8), one "Van Accessible" sign and signpost. Parking lot informational signage will be handled by the City under a separate contract. All items referred to above shall be included in the quoted cost. All material quantities must be verified by the City Director of Project Management (material tickets). The Contractor receiving the award shall furnish an approved performance bond, payment bond and written proof of insurance with the City listed as an additional insured before receiving Notice to Proceed. Contractor shall complete and submit Daily Report Forms to the City's Construction Manager. Invoices will "NOT" be processed until the completed Daily Report Forms are submitted.
