The County is not responsible for delays in the delivery of mail by the U.S. Postal Services, private couriers, or the County's mail system. It is the sole responsibility of the bidder to ensure that its bid reaches the County's Procurement office by the designated date and hour. Bid receipts and openings or the receipt of Bids scheduled during a period of suspended business operations will be rescheduled for processing at the same time on the next regular business day. Bids are to include addenda or changes to a response, Bids will not be accepted via Fax machine or by Internet E-mail, orally, or by telephone . Prince George County is soliciting sealed bids from qualified bidders to establish contract (s) to provide well installation and well abandonment at 2706 River Road, Prince George VA 23875. The contractor shall furnish all labor, supervision, equipment, tools, parts, supplies, and materials, as necessary, to perform the services as described herein: Lump Sum Cost for Line Item #1 (include total cost for items A-H). These are known factors. Vendors must invoice for actual cost incurred as described herein. The initial bid should be an average cost/foot of depth for 75 feet (add other itemized costs such as mobilization/setup, etc.). Please note: 75 feet is an estimate, vendor will bill based on actual service performed, actual depth drilled (as verified on the GW-2 and VDH's Record of Inspection). Vendor will only be paid for actual well depth drilled, casing depth, grout depth, depth of drop pipe, pump, etc. Vendor must provide a detailed bid for 75 feet; any additional footage must be listed as additional services at a per foot rate. Vendors who submit bids for over or under 75 feet will not be accepted. A. Obtain a private well construction permit from the Prince George Health Department, to include the abandonment of the existing private water well, at 2706 River Road, Prince George, VA 23875. B. Construct a private water well that meets the location, construction specifications, water quantity, and water quality requirements of the Virginia Private Well Regulations (12VAC5-630 et seq., the Regulations). C. Provide and install the well pump, plumbing, and electrical services required to convey the water under sanitary conditions from the well to the home plumbing system in the quantity required by the Regulations and the Virginia Uniform Statewide Building Code. This includes obtaining all permits and inspections necessary by the local authority in compliance with the Virginia Uniform Statewide Building Code. D. Disinfect the well and plumbing as required by the Regulations and provide the required Uniform Water Well Completion Report (GW-2). Obtain final compliance from the health department and a Record of Inspection and assure that the well is tested and complies with the water quality requirements of the Regulations. E. Appropriately abandon the existing private drinking water well as specified by the Regulations and by the Prince George County Health Department Well Construction Permit. This would include disinfecting the existing well and assessing the well depth, materials, and depth to water. Salvage any materials that can be removed from the well. Additional requirements and conditions may be included in the abandonment permit. Submit the required Uniform Water Well Completion Report GW-5 Well Abandonment Form with all required information filled out. F. Follow all regulations and obtain required permits, if needed, for erosion and sediment control including stabilizing, seeding, and grading the site after construction to return the site to its original state. Measures shall be provided to control runoff.. All construction debris must be removed from the site and disposed of properly. G. Bidders shall comply with all requirements of the Department of Professional and Occupational Regulations (DPOR) for contracting and executing the contract with Prince George County. Operate within the requirements of the DPOR Water Well System Provider Certification and proof of insurance. H. The successful bidder must coordinate their work with the contractor performing the installation of a dual transmission main in this same area to avoid any conflict of activities. Itemized Additional Services (If Needed) Bidder must contact the Prince George County Engineering & Utilities Project Manager listed on the Notice to Proceed for approval prior to any additional services being performed
