Immediate family relationships, for disclosure purposes, is defined as spouse; children; parents (natural and by-law); and siblings (natural and by law). Disclosure of this information will not necessarily preclude the award of a contract to the undersigned. The undersigned party, in accordance with intent of Centerville Charter Section 12.06 and Centerville Municipal Code Section 606.17, agrees to disclose, to the best of its knowledge and ability, the following information.
