Renovation of a medical facility in Ravenna, Ohio. Completed plans call for the renovation of a 7,500-square-foot medical facility.

The bid opening will be held in the lobby of the Office of Internal Services located at the back of the Portage County Administration Building off Chestnut Street. The project consists of renovation of approximately 7,500 s.f. of office space for a new outpatient medical clinic located at 771 North Freedom Street in Ravenna, Ohio. The clinical area consists of three general examination rooms, a clinical lab, phlebotomy room, three dental operatories, dental x-ray room, dental lab, reception area and lobby, two counseling rooms, a nurses station and support spaces. Mechanical work includes installing new rooftop units and ductwork and new plumbing fixtures in the area of renovation. Electrical work includes new power, data and lighting systems in the area of renovation. Each bid must be accompanied by a Bid Guarantee in either of the following two forms: Option A - a bid bond in the full amount of the bid or Option B - a certified check or cashier's check in the sum of 10 percent of the amount of the bid. Bid security furnished in bond form shall be issued by a surety company or corporation licensed in the State of Ohio to provide said surety. The Bid Guarantee is conditioned to provide that if the Bid is accepted and awarded the Bidder will enter into a contract with the County and will file a Performance Bond as required. The Bidder to whom the Contract is awarded will be required to furnish a Performance Bond in the sum of 100 percent of the contract price for the faithful performance of the Contract. If the Bidder selects Bid Guarantee Option A, the Bidder may use this Bond as the Performance Bond. Each bid must contain the full name of the party or parties submitting the proposal and all persons interested therein. Each Bidder must submit evidence of his experience on projects of similar size and complexity. No bidder may withdraw his bid within thirty days (30) after the actual date of the opening thereof. The contractor should be aware that this project is funded with a grant using federal funds and all HUD requirements must be met.
