This RFQ is the first step in a two-step process. Firms deemed qualified will be shortlisted and may be invited to respond to provide price quotes and/or to specific Request for Proposals (RFPs) as needs arise. Qualified electricians may be asked to perform, but are not limited to, the following services: o Routine electrical maintenance and repair o Emergency electrical response o Electrical system upgrades o Installation of lighting, outlets, and other fixtures o Support for capital improvement projects o Assistance with energy efficiency upgrades o Generator installation and maintenance Work may occur at municipal buildings, parks, streetscapes, public utilities, or other properties owned or maintained by the Municipality Interested firms must meet the following minimum qualifications: o Valid Ohio electrical contractor's license o Minimum five (5) years of experience providing electrical services to public agencies or municipalities, or like entities o Proof of insurance coverage meeting the municipality's requirements (General Liability, Workers Compensation, etc.) o Ability to comply with all applicable local, state, and federal codes and regulations At a minimum, the general statement of qualifications (SOQ) must contain the following information: 1. Cover Letter 2. Company Profile 3. List of Key Personnel and Resumes 4. Relevant Experience with Similar Clients 5. Three (3) References 6. Copy of Electrical License(s) 7. Proof of Insurance 8. Rate Schedule (optional if not requested in RFP phase) Any statement of general qualifications that does not contain all of this information may be deemed not prequalified at the discretion of the City of Fairview Park. All statements of general qualifications are required to be a maximum of five (5) pages in length (not including Proof of Insurance). Submissions will be evaluated based on: o Experience and qualifications of the firm and personnel o Quality and relevance of past work o Responsiveness and completeness of the SOQ o References and reputation o Availability and capacity to perform the work This RFQ does not commit the Municipality to award a contract or pay any costs incurred in the preparation of responses. The Municipality reserves the right to reject any or all SOQs, or to waive informalities and irregularities. Inclusion on the prequalified list does not guarantee future work.
