Description or Work: Construction on an undeveloped site of 100 feet x 117 feet at 715 Main, Fairplay, CO. The building has a footprint of the first level Pharmacy of 2,256 Sq. Ft. The second level has a one bedroom suite of 936 Sq. Ft. and a two bedroom suite of 1320 Sq. Ft. Not included in the Sq. Ft. are the stairs and decks. The building is wood frame. Type V construction. There is a 2 hour separation between the lower level and upper level. There is a one hour wall separation between the upper level suites. The Furniture, Fixtures, and Equipment shown on Pharmacy level is not part of this bid and is provided by others. The appliances in the upper level suites are not part of this bid. The CDOT driveway approach is part of this bid. Pavement of the driveway, parking lot, and parking circulation and other site work are part of this bid. Landscaping planting is not part of this bid. Area to be landscaped is to be left at final grade for landscaping by others. Tax Exempt: This project is Tax Exempt. A Tax Exempt Certificate will be provided by South Park Health Service District. Performance and Payment Bond: In accordance with Colorado law, a Performance and Payment Bond is required. Upon award of contract, selected contractor must deliver a payment bond (in form supplied by District) and a performance bond (in form supp lied by District) in an amount not less that on-half the total amount payable under terms of the contract. A Bid Bond is not required. Bids will be presented to the South Park Health Service Board at 10 AM, October 22 for review. The selected bidder will enter into the form of contract provided by the District containing the normal terms and conditions for a Colorado governmental entity. South Park Health Service District reserves the right to reject any and all bids, or to waive any informalities or irregularities in bids, and to accept the bid that, in the opinion of the Board, is in the best interest of the District.
