All responses shall be mailed or hand-delivered to Madison County Fiscal Court no later. Madison County Fiscal Court 135 W. Irvine Street, 3 rd Floor - Suite 300 Richmond, KY 40475 Crutcher Pike Bridge Concrete Attn: Scott Shepherd, Road Supervisor., At Which Point They Will Be Opened and Publicly Read at the Courthouse Annex in the Madison County Judge/executives Office. The objective of this RFP is to select the most competitive and qualified vendor to provide concrete for the Crutcher Pike bridge. Mandatory Proposal Requirements: Interested vendors are requested to submit the following formation as part of their proposal: o Labor, equipment, and material to install concrete deck o Provide and install reinforcement o Place and finish concrete paving o Provide and install sealer o Provide pumping o Provide concrete.
