The work consists of the "Stuart Street and Lafayette Avenue Resurfacing and Safety Improvements Project" in the City of Woodbury, County of Gloucester, NJ. The work includes: milling of approximately 4,800 SY of existing road surface, installation of 600 tons of hot mix asphalt surface course at 2 inches thick, installation of 400 linear feet of concrete curb, and installation of 125 square yards of concrete sidewalk. Instructions to bidders include: Each proposal must be accompanied by a Proposal (Bid) Bond or Certified Check for not less than ten percent (10%) of the bid amount, not to exceed $20,000, payable to the City of Woodbury; a Non-Collusion Affidavit; and a Consent of Surety as to a Maintenance Bond per N.J.S.A. 40A:11-16.3b. Compliance with the 'Buy American' requirement and all NJAC 7:22-9.1 et seq. provisions for economically disadvantaged individuals (SEDs) is required. All blank spaces in the proposal must be filled in and no changes to the phraseology may be made. All erasures, interpolations, or other physical changes in the bid form must be initialed or signed by the bidder. The successful bidder must furnish, within ten days after award, a Surety Corporation Performance Bond or Letter of Credit equal to one hundred percent (100%) of the contract price. All bonds or letters of credit must be written by a surety company listed on the Federal Treasury List and authorized to do business in New Jersey. Compliance with all applicable New Jersey laws and regulations including Affirmative Action, Prevailing Wage Act, Americans with Disabilities Act, Disclosure of Stockholders, Public Works Contractor Registration Act, and New Jersey Business Registration requirements is mandatory. Bids must be prepared on standard forms provided, enclosed in a sealed envelope labeled for the project, and addressed to the City Clerk.
