Provide preventative maintenance, inspections, and 24-hour emergency repair service on HVAC systems. Proposals must include price per unit for preventative maintenance as requested on the Quotation Sheet (Exhibit #2), averaged across all units listed. The Quotation Sheet must be completed in its entirety. The Questionnaire (Exhibit 3) should also be completed in full and submitted with the proposal. Proposals must be valid for 90 days and prices should exclude sales or usage taxes. All proposals shall be notarized. The evaluation will consider the ability to provide standard HVAC equipment and repairs, price, service promptness and response time, character and reputation, experience and efficiency, and proximity to the operations center. North Carolina contractor licensing statutes apply. Trade secret information should be submitted separately and marked accordingly.
