Scope of Work: The work involves electrical troubleshooting and repairs. Instructions to Bidders: - Bids must be submitted in duplicate, with one exact digital copy. - All required documents must be enclosed, including a signed document requirements checklist, completed bid form, insurance certificates (naming the City of Atlantic Beach as Certificate Holder and evidencing required coverage), three positive references (excluding the City of Atlantic Beach as a reference), electrical contractor license(s), documentation of Master Electrician certification in Industrial Motor Control for Water/Wastewater Treatment Facilities, proof of graduation from a State approved apprenticeship and training in industrial electricity, a client list with at least 5 years’ experience with Water/Wastewater facilities, proof of business tax receipt, and the following completed and signed forms: - Agree to Base Bid Specifications Form - Anti-Kickback Affidavit - Bidder’s Qualification Statement - Conflict Affidavit - Contractor Certification and Addendum Acknowledgement - Drug-Free Workplace Certification - Equal Opportunity Statement - E-Verify Form - Exceptions to Solicitation Form - New Vendor Information Request Form - Non-Collusion Certification - Public Entity Crime Affidavit - Public Records Provisions Form - Subcontractor Participation Schedule - Vendor Certification Regarding Discriminatory Vendor Lists - W-9 Request for Taxpayer Identification Number and Certification - Bid prices must remain valid for ninety days after the public opening. - Goods and services proposed must meet all requirements of the City of Atlantic Beach ordinances. - The City reserves the right to reject any or all bids, waive informalities/technicalities, and make awards in whole or in part as deemed in the City's best interests. - Entities or individuals on the convicted vendor or discriminatory vendor lists are prohibited from bidding or performing work as described in applicable Florida Statutes and House Bill provisions.
