Site work and paving for a sidewalk / parking lot in Bloomington, Indiana. Completed plans call for site work for a one-story above grade sidewalk / parking lot.
This project includes, but is not limited to, the construction of curb ramps, curbs and sidewalks at five intersections along east 2nd St. The installation of a new storm inlet and pipe is included in this project. Bids are to be submitted in proper form as described in the Instructions to Bidders. Each Bidder must include with their sealed bid: a properly executed Questionnaire Form 96 of the State Board of Accounts; a cashier's check or certified check drawn on an acceptable bank or a Bid bond equal to five percent of the total amount of bid for a public works project estimated to cost at least $200,000; a properly executed Trench Safety Systems Affidavit if the project may require creation of a trench of at least five feet in depth; a copy of the bidder's written plan for an employee drug testing program to test the employees of the bidder for drugs if the estimated cost of the public works project is $150,000 or more; and a signed contract compliance certification form. For projects utilizing Federal funding, wage rates shall be in compliance with the Davis Bacon Act. For bids of $200,000.00 or more, the successful bidder shall furnish both a performance bond and a payment bond for 100% of the contract amount prior to execution of the contract, and the bonds shall remain in effect for a period of one year after final acceptance of the work. Each bidder must ensure, to the greatest extent feasible, that opportunities for training and employment are given to lower-income residents of the project area and that purchases and/or contracts for work in connection with the project are awarded to small business concerns located in, or owned in substantial part, by persons residing in the area of the project. The City of Bloomington is an equal opportunity employer, and bidders must comply with all equal employment requirements under Title VII of the 1964 Civil Rights Act as amended and under the Bloomington Human Rights Ordinance as amended. For proposals over $10,000.00, a signed contract compliance certification form must be submitted as part of the bid packet by the bid deadline. Bidders who fail to submit the contract compliance certification form by the bid deadline are subject to disqualification. Bidders must comply with the City's contract compliance policy covering nondiscrimination of protected classes, anti-harassment, grievance processes, and prohibition of retaliation. Contractors considered covered employers are required to pay their covered employees at least a living wage as defined by the Bloomington Living Wage Ordinance. If the project may require creation of a trench of at least five feet in depth, the low bidder shall submit a trench safety plan to the project engineer at least ten days prior to beginning work. The Board of Public Works reserves the right to waive any informality and accept or reject any or all bids submitted. Bids may be held for up to sixty days from the date of opening for review and investigation of qualifications before awarding the contract.
