This project includes, but is not limited to, resurfacing and re-striping three parking lots located in Bloomington, Indiana, specifically Lot #1 at 4th & Dunn, Lot #3 at 4th & Washington, and Lot #5 at 6th & Lincoln. Bidders are required to submit their bids electronically in proper form. Each bidder must include a properly executed Questionnaire Form 96 of the State Board of Accounts; a cashier's check, certified check, or bid bond equal to five percent of the total bid amount (if the project is estimated to cost at least $200,000); a properly executed Trench Safety Systems Affidavit if applicable; a written plan for employee drug testing if the project is estimated to cost $150,000 or more; a signed contract compliance certification form; and properly executed Non-Collusion and E-Verify Affidavits. For bids of $200,000 or more, the successful bidder must provide both a performance bond and a payment bond for 100% of the contract amount, which remain in effect for one year after final acceptance of the work. Bidders must comply with all requirements for equal employment and the Bloomington Living Wage Ordinance, and meet contract compliance policy requirements for nondiscrimination, anti-harassment, grievance processes, and prohibition of retaliation. If required by project scope and Indiana state law, bidders must submit a pre-qualification with the Indiana Department of Transportation and provide a trench safety plan at least ten days before starting work if trenches over five feet are involved.
