Plumbers for Replacement of Lead Service Lines The primary objective of this RFQ is to identify qualified and interested Offerors who have the expertise, experience, resources, and capabilities to replace lead service lines on private property. The project work will consist of replacing LSLs with new copper from the public right-of-way line (near the curb stop valve location) to approximately 6-inches beyond the meter setting located inside the house/building, including installing a new meter setting according to GCWW Rules and Regulations. Work also includes patching pipe penetrations, backfilling, yard and paved surfaces restoration and related work. Additionally, Offerors will be required to perform a Pre-Inspection prior to the lead service line replacement. If copper service line material is discovered at the meter setting, caution is recommended for excavating at the right-of-way. If copper service line material is discovered at the right-of-way, two picture documentation submittals will be required to receive payment. A not-to-exceed predetermined right-of-way cost will be the payment for discovering copper service line material and not performing the lead service line replacement. For Private Side Only Pre-qualification: 1. Describe expertise and experience in the use of trenchless technology methods to install water service lines, such as the "cable" or "boring" method. 2. Describe expertise and experience in open-cut trench installation methods for water service line installations. 3. List of equipment owned or rented that will be used to complete the work described above. 4. List of similar water service line installations or replacements performed in the last three (3) years. 5. List of any employee(s) who is a licensed plumber with the State of Ohio or another state plumbing licensing board. 6. List the number of employees on staff to perform the work described above. 7. Provide proof of liability insurance and workers' compensation insurance with the State of Ohio. (Please refer to Purch Form 160 attached) 8. Indicate if the Offeror has performance and payment bonding capability and corresponding amounts. For Full (main to meter) Pre-Qualification, must include all Private Side Only items, plus those listed below: 1. List prior experience working within the public right-of-way and creating Maintenance of Traffic (MOT) plans. 2. List prior experience performing a service tap on a water main under full pressure. A minimum of 50 taps must have been performed in the past 12 months in the GCWW system. 3. List prior experience installing a minimum of 1000 feet of copper piping in the GCWW system within the past 12 months. NOTE: The above criteria will be used to determine the firms who will qualify on the Pre- Qualified List. Contractor shall be responsible for the following: Planning/Site Prep o In locations that will receive public side replacement, the party performing the public side replacement will pay for and obtain a Right-of-Way permit from the appropriate roadway agency. Costs associated with the permit shall be included in the public side replacement bid item. o Coordination of this work with the property owners will be performed by the contractor. GCWW will provide customer contact information at the time of the notice to proceed. The Contractor shall make at least three attempts to schedule the replacement using the contact information provided. If experiencing issues contacting the customer, GCWW may assist in contact. o Prior to commencing the work, the Contractor shall inspect the existing service line at the curb stop and at the interior entry point to the building. If the Contractor determines that the existing service line is copper at both locations, Contractor shall contact the GCWW Inspector, and if confirmed by GCWW, replacement work shall terminate. Contractor shall provide pictures of both inspection locations for verifications. If the City and Contractor verifies that the existing service line is lead, brass, galvanized steel or other non-lead-free material, contractor shall continue with the replacement work. Payment for investigation efforts at locations where copper is found will be in the amount of the ROW bid item to compensate for inspection and restoration. If ROW restoration is not included in the scope, then compensation may be negotiated with GCWW to compensate for interior inspection visit. o Call the Ohio Utilities Protection Service (1-800-362-2764) at least 48 hours before any excavation is performed. The Contractor shall be responsible to locate all utilities within the proposed work area on private property. This includes the responsibility of hiring a 3 rd party utility locator as needed. Damage caused by the lead service line replacement shall be responsibility of the Contractor to repair or replace in compliance with Utility requirements at no additional cost to GCWW. o Contractor shall post any "No Parking" signage necessary to perform the work. o Provide weekly replacement schedule to the assigned GCWW Inspector, providing at least 24-hour notice before a replacement. o Prior to beginning work, the contractor shall secure the following spare parts and make available on the crew vehicle at the time of the replacement. Use of these parts is in case of emergency to avoid extended water outage and will be compensated for via negotiated change order if use was necessary. o Required Spare Parts for Public Side Work: 6" leak clamp with a 3/4 " outlet 8" leak clamp with a 3/4 " outlet 3/4 " and 1" ferrule 3/4 ", 1", and two smaller GCWW-specific size ferrule adapters with rubber gaskets 3/4 " and 1" Yellow compression x flare fitting (aka lead pack) o Required Spare Parts for Private Side Work: 3/4 " and 1" curb stops and insulators Adjustable length curb box for 3/4 " and 1" curb stops.
