Each bid must include a 5% bid bond or certified check, a sworn and notarized criminal background affidavit, a sworn and notarized familial relationship disclosure statement, and a sworn and notarized Iran Disclosure Statement. Bids missing any of these required disclosure statements will not be accepted.
