Site work and paving for a road / highway in Milwaukee, Wisconsin. Completed plans call for site work for a road / highway; and for paving for a road / highway.
Financing for work under this contract is provided through the City of Milwaukee, Neighborhood Improvement Development Corporation (NIDC) who, through its loan agreement with the Owner, reserves certain rights and privileges as Lender, which include but are not limited to: development of the scope of work, assistance with the bid process, preparation of the Contract, approval of change orders and payments, and other aspects associated with contract execution and implementation. The work presented in this project is to be started within 10 days after Contractor's receipt of a Notice to Proceed and is to be satisfactorily completed within 60 days thereafter. Interior/Exterior Completion and/or Weather-Affected repairs, if any, must be completed as further outlined and incorporated in Bid Attachments A and/or B respectively. (Strike if not applicable.) The Owner reserves the right to reject any and all bids and to waive informalities. It is NIDC policy that, if: (a) the Contractor's proposal/bid complies with the specifications and other requirements, and (b) the Contractor meets NIDC's minimum requirements as outlined herein, and (c) the selected bid is reasonably within range of the market price for the work, then Owner may select any Contractor who, in Owner's opinion, is the most responsible and responsive bidder. 1 Fence Repairs: Replace the bent corner post at the Northwest corner of the property. Replace the bent top rails. Re-secure the fencing. CV 2 Site Stair Handrail: Install a code compliant handrail at the front walkway stair. CV 3 Tree Trimming: A.) Remove the trees adjacent to the foundation at the East side of the house, down to grade. B.) Cut back all other the tree/s limbs which are near the roof and siding, to create a clear distance of 10' from the house. Cut off these branches at the main trunk. CV HOUSE 4 Roof Edge Repairs: There are two noted areas with rotten deck boards and broken/rotten/damaged rafter tails. Replace the 3/4" deck boards and install new dimensional lumber to support the roof edges at these locations. Replace fascia boards, if that is also necessary. CV, Pb-N 5 Chimney Removal: Remove the chimney to below the roof deck, and deck-over. CV 6 Roof Replacement: Tear off all layers of existing shingles and replace the entire roof, and all smaller side roof surfaces. (Note: There are multiple layers of shingles, including the original cedar shingles.) Replace any underlying rotten or damaged roof sheathing, and install 7/16" OSB sheathing over the entire deck. Provide ice and water shield a minimum of 3 feet wide at all roof edges, install synthetic roofing felts, and metal drip and apron flashing. Install new roof vents per code. Provide all necessary flashings, and minimum 30 year dimensional shingles. The owner shall make the shingle color selection. 7 New Roof Warranty: The contractor shall apply for the shingle manufacturers roof warranty, or provide all information and assistance necessary (material purchases, date of purchase, warranty forms/application etc.) for the owner to apply for the shingle warranty. The contractor is to install the required products as necessary to establish the manufacturers warranty; starter strips, flashings, underlayment, roof vents, etc. All materials shall be installed according to manufacturers requirements. NOTE 8 Soffit and Fascia Wrap: Repair or replace all damaged fascia, gable, and soffit board wraps. Color is to be white (match). CV, Pb-N 9 Front Entry Porch (wood): A.) Remove and replace the porch structure (joists, etc.), railings, and tongue and groove decking. Install additional 4x4 posts with frost proof footings as necessary for support. Install perimeter trim boards, and skirting. B.) Replace the porch stairs with new code compliant wood stairs. Install wood guardrails at the porch perimeter, and guardrails with finished (painted) handrails at each side of the stairs. C.) All construction is to be code compliant. PERMIT, CV, Pb-N 10 Jump Porch: Remove and replace the entire structure (including wall bracket supports), and railings. Use treated wood construction. All construction is to be code compliant. PERMIT, CV, Pb-N 11 Siding Repairs: Repair or replace all loose or damaged existing siding and replace with close-matching siding. (GAF WeatherSide Profile12 12 in. x 24 in. Fiber-Cement Siding) Paint all repaired areas to match existing. Install corner trims, with matching trims, where missing. Where siding is being disturbed and disposed of, it is be performed by an asbestos abatement certified company. CV, Pb-N 12 Front and Rear Entry - Door Replacements: Replace the existing front and rear entry main doors with a metal clad exterior door with high upper-lites. Install a thumb-turn deadbolt and locking handle. Paint all exposed wood trim work; jambs and brick molding, etc. CV, Pb-N 13 Jump Porch - Door Replacement: A.) Install a wood header to replace the previous header which was removed. Fasten in place with 16d nails. B.) Replace the existing door with a metal clad exterior door with half upper-lite. Install a thumb-turn deadbolt and locking handle. C.) Paint all exterior exposed wood trim work; jambs and brick molding, etc. CV, Pb-N 14 Front and Rear Entry - Security Doors: A.) Replace security doors at front and rear entry's. Install the new glazed security/storm doors. Install a thumb-turn deadbolt and locking handle at each door. Door is to be keyed-alike with the main door. CV, Pb-N 15 Vinyl Replacement Windows - Lead Abatement: Replace the existing wood sash windows with matching vinyl replacement windows. Wrap all exterior exposed wood in color matched aluminum sheet metal. Install color matched sealant at all window edges,seams, and wraps to fully enclose underlying potential lead paint and make water-tight. Insulation Note: For all double hung window's which previously used counter-balance weights; remove the weights and ropes, then infill the open pockets with insulation, and apply sealant at all gaps and seams to eliminate air infiltration. (Quantity: 21 window units, field verify.) CV, Pb-A 16 Basement Window Panels: Remove any loose paint, then prime paint and finish paint the panels, color per owner. 17 Lead (Pb) Clearance: The contractor is to contact and coordinate with the Milwaukee Health Department to obtain a permit, and submit lead testing clearances. The contractor is to conduct all necessary cleaning at the property in order to obtain passing lead swipe tests. Pb-N 18 Plaster and Paint Repairs: Remove loose plaster and paint at all areas which possibly contribute to lead (Pb) contamination. All patches are to match the existing finish of the adjacent surfaces. Spot prime paint and finish paint the patched areas. It is the contractor's responsibility to locate all such repairs, which are required to obtain a visual inspection lead clearance. (All friable potential lead paint is to be encapsulated.) CV, Pb-N 19 Alarms: Install a combination smoke/CO alarm in the basement, first floor, and attic. Install smoke alarms in each bedroom. All alarms shall be sealed battery, 10 year lithium ion units. CV 20 Electrical Cover Plates: Install outlet,switch, and junction box cover plates at all locations where missing. CV BASEMENT 21 Stair Repairs: Remove the existing riser and tread finishes at the stairs and rear-entry landing. Replace the broken treads. Install new finishes. CV 22 Laundry Sink Repair: Replace the faucet. CV 23 Electrical Repairs: A.) Remove the abandoned receptacles/conduit to the North of the electrical panel. B.) Correct the loose junction box, hanging in front of another junction box, just to the South of the electrical panel. Open and correct this box - or remove the box and the conduit if it is abandoned. C.) There is a junction box above the center beam, near the middle of the basement, with a BX conduit connecting to knob-andtube cloth wiring outside of the box. Correct all wiring to a code compliant state. PERMIT, CV 24 Water Heater Repair: Correct the condensate trap off of the exhaust fan. It is pinched, and the trap loop should be secured. CV FIRST FLOOR 25 Kitchen Sink Drain: Repair the link on the PVC solvent-welded drain connection/s. CV 26 Kitchen Flooring: (Kitchen and back stair top landing.) Remove all layers of floor finish down to the base wood floor finish. Install a 1/4" smooth luan plywood subfloor, and sheet vinyl flooring. Install finished shoe moldings at all wall edges, and transition thresholds at all other edges. CV 27 Bathroom Flooring: Remove and replace the bathroom flooring, per the same requirements as Work Item #26. CV 28 Walls and Shower Surround: A.) Remove the plaster on all walls and ceiling with water and mold/mildew resistant drywall. B.) Install batt insulation and vapor barrier at the exterior wall and in the ceiling joists. C.) Prime paint and finish paint all walls and the ceiling. D.) Install new finished wall base moldings. E.) Install a new shower surround and ceramic tiles wall finish. Apply a waterproofing coating on the underlying wall panels (Redgard, or approved alternate.) F.) Install wood support blocking, then install two towel holders, a robe hook, a shower rod, a medicine cabinet/mirror, and a toilet paper holder. Replace the HVAC grille. Owner is to pre-approve all selections. CV, Pb-N 29 Plumbing Fixtures: Replace the toilet. Replace the pedestal sink and faucet. Replace the shower valve, tub filler and shower head. Use owner approved fixtures for all. 30 Bathroom Electrical: A.) Install a new 3-bulb (minimum) light fixture. B.) Clean the exhaust fan. CV, PERMIT 31 Electrical Repairs: A.) Kitchen: Replace the existing receptacles with GFCI's at the kitchen sink counter. B.) Kitchen: Correct the light switches and outlet (2-gang box) adjacent to the kitchen stove. The switch works intermittently, and receptacle is not grounded. C.) Back Stairway: Properly terminate the wires at the former wall light fixture within and junction box and cover plate. SECOND FLOOR 32 Electrical Repairs: Replace the exposed knob-and-tube wiring with code compliant wiring. (BX, conduit, etc.) ALTERNATE WORK ITEMS ALT-1 Radon Mitigation System: Install a complete radon mitigation system consisting of a collection basin/piping, vent piping, exhaust fan, and pressure monitoring device.
