The scope of work includes but is not limited to the replacement of the existing storefronts and doors at Buildings 1 & 2 of Brooklyn Children PC. The estimated construction cost range is between $1,600,000.00 and $1,750,000. Only those Contract Documents obtained in this manner will enable a prospective bidder to be identified as an official plan holder of record. DASNY takes no responsibility for the completeness of Contract Documents obtained from other sources, which may not be accurate or may not contain all issued addenda. - Prospective bidders are advised that the Contract Documents for this Project contain new "GENERAL CONDITIONS for CONSTRUCTION" dated June 17, 2021, with significant revisions. Review applicable sections for potential impact on bid price prior to submittal. - To become a registered planholder and to be placed on the bidder's list, purchase of a digital download of the Contract Documents is required. Printed sets are also available for an additional cost. Purchases are non-refundable and non-returnable. - For mail orders, include company name, physical address, contact person, phone number, email, company fax number, FedEx or UPS shipping account number, and Federal ID number. If no shipping account is provided, include an additional non-refundable check for shipping. - The bidder or its principals must have a minimum of five years' experience in similar work and provide a list of five contracts of similar size, scope, and complexity completed or substantially completed within the last ten years on the Minimum Qualifications Form. Projects with complex specifications may require more experience. - Requirements include Contract Security, EEO/AA Insurance, NY State Vendor Responsibility Questionnaire (Executive Order 125), and compliance with relevant New York State laws and business registration requirements.
