The project is for lead reduction services at 137 E Lake St, Sand Lake, MI 49343. The work involves addressing lead-based paint hazards as specified in the attached Lead Inspection and Risk Assessment (LIRA) Report and Work Write-Up, which prescribe the abatement methods, interim controls, or a combination of both. Completion of a lead clearance examination by the same third-party that performed the LIRA is required for all impacted work areas. This is to ensure no settled dust-lead or soil-lead hazards exist post-activity. Not all hazards identified in the LIRA may be addressed in the project. Only professionals holding an active State of Michigan Lead Abatement Firm certification who employ at least one Lead Abatement Supervisor and the necessary number of Lead Abatement Workers are eligible. Submission must include proof of this certification. For lead abatement activities, prior notification to the Michigan Department of Health and Human Services is mandatory, with documentation submitted to Kent County. Required documents to remain on-site during the project include the LIRA Report, Work Write-Up with any approved change orders, and the Occupant Protection Plan approved by Kent County.
