Demolition, site work, paving and new construction of a civil project in Hebron, Connecticut. Completed plans call for the demolition of a bridge / tunnel; for the construction of a bridge / tunnel; for site work for a bridge / tunnel; for paving for a bridge / tunnel; road / highway; and road / highway.
Prospective bidders must have a current sworn Statement (CON-16) on file with the Connecticut Department of Transportation and be prequalified to perform Group No. 8 - Minor Bridges work. The Bidder's Prequalification approval letter signed by the CTDOT Contracts Manager shall be included as part of the bid package submitted to the Municipality. Any Contractor or Subcontractor engaged in surface preparation and/or coating application must be certified by the Steel Structures Painting Council. A Surety Company Bond, on the form furnished by the Municipality, for at least 30 percent of the amount of the bid must accompany each proposal. A certified check will not be accepted. The Municipality reserves the right to reject any and all bids. CONTRACTORS that find discrepancies and/or errors in or between plans, specifications, quantities and other matters, must immediately notify Andrew J. Tierney, Hebron Town Manager in writing not less than ten days before the scheduled bid opening The existing bridge will be completely replaced with a new single 42-foot clear span bridge with a superstructure consisting of precast concrete beams and integral abutments founded on micropiles socketed into bedrock. We understand that the new bridge will be constructed within the same general area as the existing bridge as shown on Figure 2, the Subsurface Exploration Location Plan. Under these items, the Contractor shall prepare and implement a vibration and movement monitoring control plan during construction for protection of buildings and property and provide pre- and post-condition surveys of identified sites. Vibration producing activities such as demolition, pile installation, vibratory compaction, pavement breaking and operation of heavy construction equipment are expected to be required for construction of this project. Work under these items include: 1. Vibration and Movement Monitoring Plan. a. Preparing a vibration and movement monitoring plan. i. Install vibration monitoring sensors. ii. Install crack monitoring gages. b. Conduct monitoring and take readings before, during and after anticipated vibration or movement producing activities including but not limited to: i. Site excavation activities and demolition including pavement breaking; ii. Cutting and demolition associated with removal of existing substructure; iii. Installation of driven or vibratory piles; iv. Vibratory compaction; and v. Until sufficient information has been collected to prove that detected movement has stopped. 2. Additional protective measures as required to prevent damage to existing buildings, appurtenances, structures or features within the influence of any construction related activities, including construction-induced vibrations and movement. 3. Structure/Building Condition Surveys and Reports. Performing surveys and preparing associated reports: a) prior to commencement of work; b) after completion of work; c) and at other locations and times during construction as directed by the Engineer. Condition surveys shall document the existing conditions of the buildings including but not limited to cracks, settlements, holes or other deterioration. Condition surveys shall also document the condition of retaining walls, pavement and sidewalks. Minimum locations requiring condition surveys and vibration and movement monitoring include: 1. Site No. 1: 330 Old Colchester Road, Hebron CT Freeman Companies, LLC performed the following tasks: o Reviewed test boring logs of exploration performed and logged by others; o Retrieved stored soil and bedrock samples for observation and analyses; o Arranged for a testing laboratory to conduct laboratory soil and bedrock tests; o Evaluated subsurface conditions; o Prepared this report containing geotechnical design recommendations and construction considerations. Section 1.04 is supplemented and amended as follows: Throughout this Section, make the following substitutions for all occurrences of the word(s) identified below for substitution: Substitute "Engineer" for "Department's Assistant District Engineer". Substitute "Municipality" for "Department" and for "State". 1.04.05 - Extra Work: is amended as follows: Add the following after the fourth sentence: Bonding costs shall not be included in the contractor's compensation request. However, if the contractor incurs or will incur increased bonding costs related to the extra work, the contractor shall request separate compensation for such costs. The contractor's request shall be itemized and include a certified statement from the bonding company stating that the value of the work will require an increase in bonding coverage and shall detail the additional costs (within allowable contract amount limitations). If satisfactory substantiation is provided, a new item for increased bonding costs will be incorporated into the contract by means of a construction order. Insert the following immediately following "Department's Assistant District Engineer" near the bottom of the paragraph: "and/or the Municipality's authorized representative".
