Renovation of a medical facility in North Rose, New York. Completed plans call for the renovation of a medical facility.
(Bathroom Renovation Designs). The vendor shall provide all labor and materials necessary to complete the work in a professional manner according to scope of work and architectural drawings if supplied. Work is to be completed within 30 days of award, unless modified below. This is a NYS Public Works project, and prevailing wages are to be paid. Refer to attached specification for the following scope of work. Upon completion of (Bathroom Renovation Designs), contractor is to schedule a quality control and quality assurance inspection with Plant Superintendent. SCOPE OF WORK includes, but is not necessarily limited to the following: 1. Designs and specifications of Bathroom Renovation for the Upstairs and downstairs bathrooms. 2. Product Specification Data. a. Construction details, material description, dimensions, profiles, finishes, and applicable schematics, and cut sheets. 3. All Designs and Specification documents need to be provided on Adobe (.pdf). 4. Document Submissions to be provided to OPWDD Point of Contact (POC) at routine Intervals: a. 60% Completion, b. 90% Completion (Issued for Permitting- signed and stamped), c. 100% Completion 5. Internal Construction Permitting to be completed by the Maintenance Supervisor with necessary OPWDD Agency Personnel in Central Office. 6. Design Vendor shall provide five (5) sets of 24"x36" blueprints upon completion, stamped/signed, and mailed to POC listed above. 7. See Specifications for OPWDD Epoxy Flooring Spec for DUR-A-Flex Epoxy floors Provided with Quote Package for flooring Specs. 8. Design Services shall also include: a. A Probable Construction Cost Estimate, based on current pricing, materials and supplies. SCOPE OF WORK FLOORING SPECS includes, but is not necessarily limited to the following: 1. Remove existing toilet, toilet and bath accessories, mirror(s), casework, miscellaneous plumbing fixtures and other accessories completely. Prior to disposal, coordinate with House Director for any items that may be salvaged. 2. Remove and replace the entire floor system, down to the existing floor joists. Examine substrate and conditions. Replace all damaged or deteriorated wood as requires, including any damaged or deteriorated floor joists. Do not proceed with installation until conditions are corrected. Standard lumber is specified for any joist replacement. Any needed floor joist replacement will result in a change order at a cost of $75.00 for each up to 8'-0" sections and $150.00 for each 8'-0" to 14'-0" sections. Provide and install two (2) layers of 3/4 " marine-grade plywood subflooring, glued and screwed to floor framing. 3. Provide and install new resinous flooring system with integral base as per "Specification Section 096726 Quartz Flooring" attached. a. Color: As selected by House Director. 4. Provide and Install New Toilet Mounting Hardware and reinstall Toilet into Bathroom, ensuring all plumbing connections are leak free. 5. Commence work within 30 Calendar days following receipt of project authorization. 6. All work will be done in accordance with approved house safety plan for project.
