The project seeks to provide workforce development for local public health agencies and community-based organization grantees, build statewide knowledge and capacity to reduce health disparities, build and share power with communities, and incorporate elements of equitable implementation. The project focuses on reducing health disparities and advancing health for populations experiencing higher disease burden. Training and technical assistance will be delivered to grantees, developing necessary tools and resources, and centering the voice and leadership of communities and organizations with an emphasis on community assets, strengths, and resilience. Activities include site visits, live training, individualized technical assistance, participation in conferences, webinars, and other communication methods to increase grantee knowledge and skills regarding health disparities, root causes, best practices, and effective implementation. The project works within an existing technical assistance ecosystem to implement strategic plan goals and objectives. Offerors must demonstrate experience as subject matter experts in health equity, have strong partnerships with identified priority populations, and show past performance in similar projects. Proposals must include an executive summary, resumes, relevant experience, work plan, budget, and references. All work must comply with accessibility requirements and state technology standards. The project total budget is not expected to exceed $720,000 for three years, with potential renewal. Proposals must be submitted as specified, and all proposal content, including pricing, must be binding for at least 180 days from submission. Additional instructions include compliance with confidentiality requirements, conflict of interest disclosures, and adherence to state procurement rules and contract terms. Offerors are not to issue news releases or announcements without state approval, and any protests regarding the solicitation or award must follow established procedures.
