Warren Consolidated Schools will accept sealed bids for Self-Provisioned Fiber Maintenance projects that comply with the outlined requirements, including Bidder Instructions, General Conditions, Universal Service Fund (USF) Conditions, Scope of Work, Timeline Requirements, Awarded Contract Requirements, and Products and Services Specifications. Bidders must submit an Intent to Bid Form, answer any major addendum questions by the deadline, and provide bids in duplicate in a sealed envelope, as well as upload an electronic copy using the specified bidder portal. No oral, telephonic, e-mail, or facsimile bids will be considered, and no bids will be accepted after the specified deadline. There is no bid bond required for this RFP. Bids must include a signed Bid Signature Page, Bill of Materials, Familial Disclosure Affidavit, Iran Linked Business Affidavit, and at least four references for similar work performed within the past three years, with at least one reference being a school or school district. The bid must identify the legal status of the bidder and be signed by an authorized representative. The Board retains the right to reject any or all bids and to qualify or disqualify bidders based on their ability to perform the required work and the suitability of their proposed products and services. Bidders are responsible for understanding all requirements and may request blueprints/drawings as needed. Any variance from specifications must be explained in writing. All quoted prices must be on a unit price basis and cannot change due to order quantity adjustments by the District. Substitutions of products or services are allowed only at no additional cost and with district approval. The selected contractor is required to keep premises clean and remove all work-related debris upon project completion.
