Contractor shall furnish all equipment and supplies needed to carry out preventative maintenance services as specified, at no additional charge. All contractor equipment must conform to OSHA regulations, and the contractor shall remove any supplies or equipment deemed not up to code. Material Safety Data Sheets for all products must be provided and maintained onsite. Any damage to equipment, finishes, or building elements must be reported immediately. The contractor must have sufficient qualified personnel and provide adequate supervision. Only workers skilled in their assigned tasks may be used, and the State may require reassignment of any such employee. Work must be performed to maintain occupant safety and minimize disruption. Removal and disposal of materials and debris must comply with regulations. All vertical transportation equipment must be maintained in optimum operating condition to ensure proper operation, following ANSI A17.1--2007 or the latest edition and all applicable codes. Full Maintenance Service includes service, repair, part replacements, callback service, and preventative maintenance. The contractor must systematically examine, adjust, lubricate, clean, repair, or replace all covered equipment. State-regulated elevators must be serviced at least once per month or per manufacturer's schedule; lifts and dumbwaiters at least six times per year. Contractor must maintain records of all service in each machine room and provide written or online reports for each visit or test. Emergency service must be provided with the contractor responding immediately for life-threatening situations, within three hours if equipment performance is compromised, and within 24 hours otherwise. The Owner reserves the right to add or remove equipment from the contract and to seek competitive bids for non-contract work. The contract duration is one year, with potential renewals for up to three additional years. Instructions to bidders: Submit quotations using the required form, with original signatures, and list add/deduct alternates as specified. Bidders are encouraged to visit the site before submitting a quotation. Quotations must clearly identify the bidder and meet all specified submittal requirements. The contractor must perform at least 15% of the contract value with its own forces. Bids may be rejected for failure to comply with submission requirements or for including unauthorized changes or conditions. Prior to contract execution, the State will notify the successful bidder, who must return the signed contract within ten days. Contractors must register with the Department of Administration if not already registered, and a completed W-9 is required before purchase order issuance.
