The work consists of the replacement of sections of roofing at Goshen Junior High School and Goshen High School as indicated in the construction documents. Bids must be properly and completely executed on Indiana State Board of Accounts, Form 96, Revised 2013, with all supplementary bid forms completed. Submit all original bid information and two complete copies. The entire bid form, including the Non-Collusion Affidavit and Bidders Financial Statement as part of Indiana Form 96, must be completed. Include a Bid Bond or certified check for 10% of the bid price. Awarded bidders must provide a Performance Bond and Labor and Materials Payment Bond in the amount of 100% of the contract, with all bond and insurance costs included in the bid amount. Each bid requires a signed and completed Non-Discrimination Affidavit (or MBE-1) as per any relevant Minority Business Participation Policy and Rules. Clearly mark each bid envelope "Roofing Bid: Goshen Community Schools, (Goshen High School and/or Goshen Junior High School)" with the name of the Roofing Contractor and project number. Bids are valid for 90 days. The contract will be awarded to the best responsible qualified bidder(s). Faxed or emailed bids are not accepted.
