Site work and new construction of a medical facility in Cut Off, Louisiana. Design plans call for the construction of a medical facility; and for site work for a medical facility.

Questions should be addressed in writing to Justin Cortez and submitted via email to justin@ghcarchitects.com Qualification Statements shall be organized in accordance with the RFQ and related documents and instructions. Qualification Statements must be as succinct as possible while completely providing all requested information. No Qualification Statements shall be reviewed until after the submission deadline specified above. The Owner reserves the right to reject any or all Qualification Statements. All proposers must be licensed Louisiana contractors under the Louisiana State Contractor's Licensing Law, La. R.S 37:2150 through 2164, as amended. As a requirement under Louisiana Revised Statute 37:2163, this Project is classified as Building Construction. As described more fully in the RFQ, funding for the development of the Project is being provided by the Louisiana Office of Community Development Disaster Recovery (LOCD-DR). LOCD-DR has received a funding grant from the U.S. Housing and Urban Development Department/Community Development Block Grant for Disaster Recovery program. The selected proposer will be required to execute the Community Development Block Grant Recovery Rider, which is included in the RFQ documents. Attention of proposers is called particularly to the requirements for conditions of employment to be observed and minimum wage rates to be paid under the contract (Davis-Bacon Act), Section 3 (Low Income Resident Participation) of the Housing and Urban Development Act of 1968, Section 109 (Non-Discrimination) of the Housing and Community Development Act of 1974, Section 503 (Non-Discrimination Against Employees with Disabilities), and Section 504 (NonDiscrimination Against Individuals with Disabilities) of the Rehabilitation Act of 1973, Segregated Facilities, and all applicable laws and regulations of the Federal government and State of Louisiana and bonding and insurance requirements. Small businesses, minority businesses, women's business enterprises, veteran-owned businesses, labor surplus area firms and/or Section 3 businesses are encouraged to participate. The proposer selected to provide CMAR services shall be required to obtain and provide a Unique Entity Identifier (UEI) prior to award; please refer to sam.gov for further information regarding the process to obtain a UEI. Proposers can view this advertisement in the Daily Comet. The method of Contractor Selection has not been Determined at this time. "Requests for proposals for design services is underway."
