Middlefield, MA 01243 Phone: 978-623-2079 Email:middlefield.assistant@gmail.com The Town of Middlefield is seeking Written Responses for the purchase and installation of playground equipment for ages 5 through 12 years of age in the center of Middlefield adjacent to the Middlefield Senior Center located at 169 Skyline Trail in Middlefield, MA 01243. The Town is also requesting, as part of the Written Response, to furnish and install required playground border timbers and stakes shown for existing swing sets and Monkey Bar Area. Measurements of the area to receive the timbers are listed below. The area will be prepared by others for the installation of the border timbers by the contractor. Safety filling of each area will also be provided and installed by others. The plan calls for the use of approved rounded bark mulch per the state required depth. The contractor does not need to provide the wood chips mulch (only to install the border). The playground contractor shall provide at least a one-year warranty on all parts and labor. Extended warranties are welcomed. Scope of Work The contractor must only use outdoor equipment, materials, furnishings, that are appropriate for children 5 to 12 years of age and must meet the needs, and development level for the age group. The contractor will be responsible for providing playground equipment based upon the enclosed Bayou Vista Playground System plan manufactured by NVB Playgrounds located at 3876 North, 700 West in Greenfield, Indiana 46140 Tel: 187-826- 2776 or approved equal. The contractor will provide erect and install all elements of the new playground system, full playground ramp, install weed preventing geodesic textile cloth barrier on all areas to receive safety wood chip mulch, and install new border timbers to hold safety wood chip mulch per the manufacturer's guidelines that are 52 inches long x 12' high x 4" wide including installation pins encompassing the new playground system and existing play areas shown on shown in the attached aerial image. The contractor does not need to provide the wood chips mulch (only to install the border). The approved design must meet and comply with Massachusetts safety, accessibility, and construction guidelines for the installation of playground equipment. The proposed equipment must include elements that also meet state accessibility guidelines. The playground will also include 1 ADA half ramp to access the ADA area of the playground. The actual location of the installation of the ramp will be determined on site. Accessible features must be able to meet a paved walk that will be constructed by others. The site for the playground will be prepped by others including the removal of existing playground equipment, supplying and installing all safety bark mulch. The playground area will be leveled, and all debris will be removed by others as part of preparing the site. Since this is a municipal project in Massachusetts, the contractor will be required to pay Commonwealth of Massachusetts Prevailing Wage Rate for all workers on the project. A copy of the rates is attached for use by contractors. The contractor will be required to furnish copies of completed Prevailing Rate paid to workers along with all requests for payment. A copy of the required form to be used is attached. All employees will have OSHA 10 training and copies of compliance cards shall be delivered to the Town at the beginning of work on the project. It is also expected that the contractor will be in compliance with any applicable Massachusetts MBE/WBE employment guidelines.
