This work consists of milling existing HMA surface, constructing WMA binder and surface course lifts, 1.75" hot in-place recycling scarification, construction of HMA IL-9.5 surface course (FG) N50 thin lift overlay at 0.75" and 1.0" butt joints, hot-in-place recycle existing HMA binder course, 3' widening of HMA shoulders, installation of new guardrails, HMA patching where needed, sinusoidal rumble strip with hot-spray thermoplastic striping installation. All proposals shall be submitted on forms furnished by McHenry County DOT. Prequalification of bidders in accordance with provisions of check sheet #LRS6 of the State of Illinois Supplemental Specifications and Recurring Special Provisions is required. Not less than the prevailing rate of wages as found by McHenry County or the Department of Labor, or as determined by the Court on Review, shall be paid to all laborers, workers, and mechanics performing work as indicated in the Special Provisions. All proposals must be accompanied by a proposal guarantee in the amount of not less than 5 percent of the bid, or as provided in check sheet #LRS6 of the Supplemental Specifications and Recurring Special Provisions. Location - Maple Street, Wilmot Road, Richardson Road, and Flat Iron Road
