The work consists of furnishing all materials and labor to resurface approximately 4,500 SY of asphalt concrete, adjust storm structures, manholes, and valve boxes to grade as necessary, and re-stripe all stop lines, crosswalks, parking stalls, and handicap symbols on Church Street. D Bidders are instructed that Contract Documents must be purchased from the office of Engineering Associates, Inc. for a cost of $75.00 per set, none of which is refundable. Bidders must register with and obtain Bidding Documents from Engineering Associates, Inc. Each bidder must provide a Bid Guaranty in accordance with Section 153.54 of the Ohio Revised Code, issued by a Surety Company licensed in Ohio. Proposals must contain the full names of all interested parties and evidence of experience on similarly sized and complex projects. Contractors and subcontractors must, to the extent practicable, use Ohio products, materials, services, and labor, and comply with equal employment opportunity requirements of the Ohio Administrative Code Chapter 123. Bidders must also comply with prevailing wage rates for Medina County and the Village of Lodi, Ohio, as determined by the Ohio Bureau of Employment Services, Wage and Hour Division. The Village of Lodi reserves the right to reject any and all bids, waive informalities, or accept any bid deemed favorable to the Village of Lodi.
