The project includes paving various streets throughout the town, which may consist of milling, cleaning, tack coat, surface and binder courses. The work to be performed and the proposal to be submitted shall include sufficient and proper sums for all general construction, mechanical installation, labor, materials, permits, licenses, insurance, and incidental requirements for the construction of the facilities. Each proposal must be accompanied by a bid guaranty in the form of a certified check or bond equal to five percent of the bid, made out to the owner. Proposals must be submitted on the forms included in the specifications and must include all information requested by Indiana Form 96 (Revised 2010), including a financial statement. The contractor awarded the work will be required to furnish a performance, maintenance, and payment bond equal to the contract amount, and all necessary insurance certificates, prior to commencing work. All bonds and insurance must remain effective for twelve months following completion and acceptance of construction. Each bidder is responsible for inspecting the project sites and thoroughly reviewing the contract documents and specifications. No conditional or qualified bids will be accepted. All applicable laws, ordinances, and regulations apply to the project throughout. Award will be made to the low, responsive, responsible bidder.
