Scope of Work to be Performed The Contractor will be required to perform prep work, removal, installation and cleanup for Pavement Preservation and Maintenance Services defined in this ITB. The Contractor shall supply labor, equipment, tools, means of transportation, traffic control, and incidentals to perform work in accordance to specifications, and to ensure a safe work environment for employees and the traveling public within the time schedule specified. 1. Scope Work under this ITB consists of furnishing all labor, materials, tools, equipment, means of transportation, and incidentals necessary to perform the services. Contractor must furnish equipment in good operating condition and operated by properly trained and qualified personnel. The Contractor must make a good faith effort to furnish the Department the fully operational equipment needed to perform Pavement Preservation and Maintenance Services at such time, and at such location(s) as directed by the Engineer. Prior to start of work, Contractor must inspect equipment to ensure the equipment is fully operational for the performance of its intended purpose. The Contractor will also be responsible for any traffic control, licenses, and permits required to satisfy the duties required herein. 2. Deliverables Delivery of Pavement Preservation and Maintenance Services shall be in accordance with the contract terms. The following deliverables are expected for work performed at the locations referenced in Exhibit 4 - Locations, Descriptions and Estimated Quantities. THIS CONTRACT CONSISTS OF SINGLE SURFACE TREATMENT AND THIN LIFT OVERLAY ON SR 112 IN TURNER COUNTY. THE PROJECT BEGINS 12 FT. EAST OF JCT OF SR 112 AND SR 90 C/L. AT EXISTING EDGELINE PAVEMENT JOINT AT LOG MILE 0.000 (MM. 20.77) (GPS COORDINATES 31.80905 N. 83.48999 W.) AND ENDING 80 FT. WEST OF TURNER/WILCOX COUNTY LINE AT LOG MILE 2.661 (MM. 23.41) (GPS COORDINATES 31.84726 N. 83.48896 W.). BEGIN FULL WIDTH VARIABLE DEPTH MILLING (TYPICAL 1 1/4 INCHES) TO MAINTAIN EXISTING ROADWAY PROFILE AND ALLOW FOR SMOOTH TRANSITION AS NOTED IN LOG MILE. BEGIN FULL WIDTH SINGLE SURFACE TREATMENT WITH NO. 7 STONE SIZE AS NOTED IN LOG MILE. BEGIN FULL WIDTH THIN LIFT INLAY/OVERLAY OF 9.5 MM SUPERPAVE AT 135 LBS. PER SQUARE YARD AS NOTED IN LOG MILE. ATTACHED ARE THE PROJECT NOTES, GENERAL NOTES, MILE LOG, SPECIAL PROVISION 150.3.03.G1&2., DETAIL FOR PAVEMENT MARKING, DETAIL FOR RAISED PAVEMENT MARKERS, DETAIL FOR SINUSOIDAL RUMBLE STRIPS AND MAP THAT APPLY TO THIS CONTRACT. PROJECT NOTES: *ONCE THE WORK COMMENCES IT SHALL BE A CONTINUOUS OPERATION UNTIL THE WORK IS COMPLETE. CONTRACTOR SHALL SUBMIT A REQUIRED PAVING PLAN FOR ALL PAVING OPERATIONS. SWEEP SINGLE SURFACE TREATMENT PRIOR TO APPLYING OVERLAY OF 9.5 MM SUPERPAVE. ALL SINGLE SURFACE TREATMENT SHALL BE COVERED WITH 9.5 MM SUPERPAVE SAME DAY. PRESSURE SENSITIVE ADHESIVE RAISED PAVEMENT MARKERS WILL NOT BE ALLOWED ON THE PROJECT. BEFORE ANY RPM'S ARE PLACED, ENSURE THAT ANY EXISTING RPM'S ARE REMOVED AND THE SURFACE IS CLEANED OF ANY DEBRIS. THIS PROJECT SHALL BE COMPLETED WITH THE USE OF A MATERIALS TRANSFER VEHICLE (MTV/SHUTTLE BUGGY) DURING PAVING OPERATION. THE CONTROLS SHALL BE REFERENCED TO GRADE WITH A SKI-TYPE DEVICE AT LEAST 30 FT (9 M) LONG WITH APPROVED NON-CONTACTING LASER OR SONAR-TYPE SKIS LISTED ON QPL 91 "GEORGIA'S LIST OF APPROVED NON-CONTACTING LASER AND SONAR-TYPE ELECTRONIC GRADE AND SLOPE CONTROLS". THE DEPARTMENT SHALL HAVE THE AUTHORITY TO SUSPEND OR STOP THE WORK AT ANY TIME IF THE WORK THREATENS THE SAFETY OF THE TRAVELING PUBLIC DUE TO THE HIGH VOLUME TRAFFIC AND/OR ANY NON-COMPLIANCE WITH CONTRACTUAL ITEMS. "BIDDER SHALL BE RESPONSIBLE FOR VERIFYING ALL QUANTITIES PRIOR TO PLACING BID AND BID APPROPRIATELY." (LISTED QUANTITIES ARE AN ESTIMATE ONLY). CONTRACTOR SHALL TURN IN TO THE MAINTENANCE CONTRACTS OFFICE THE OMR REPORT FOUND IN SOP 39 (OMR-CUP-66) 30 DAY RETRO REFLECTIVITY TESTS.
