Installation, labor, materials and equipment for rehabilitating Lift Station #5 which includes installing a new 18" riser on the valve vault, and regrading the surrounding ground to drain away from the lift station. Additional work includes installing safety grates in wet well hatch openings in Lift Station #1, 2, 3, 4, and 5. All work must follow Indian Health Service Vineland Community Lift Station #5 Rehabilitation & Safety Grate Installations Project specifications. Proposals must include a fixed fee for the scheduled work and be signed by the contractor to be valid. A copy of the Mille Lacs Band Vendor's License (or submitted application), and a current certificate of insurance for General Liability and Property Damage are required. Federal Davis Bacon wage scale applies. The prime contractor must perform at least 33.3% of the work. Compliance with the EEO executive order 11246 as amended, the Copeland anti-kickback act, and sections 103 and 107 of the contract work hours and safety standards act is mandatory. A 1/2% TERO Tax will be charged based on the final project cost. Each bid must include a BID BOND for ten percent (10%) of the total bid amount. Performance and payment bonds each for 100% of the contract are required. Only mailed proposals to the designated P.O. Box are accepted; hand-delivered or courier deliveries are not accepted. Proposals not submitted in this manner will be rejected. Firms must be licensed with the Mille Lacs Band of Ojibwe or provide proof of license application. TERO requirements apply.
