The scope of work involves removal and proper disposal of the existing basketball court surface and the existing posts and backboards, including their concrete foundations. The disturbed subbase is to be graded, compacted, and prepared for construction of the new court. New posts, backboards, and hoops are to be installed, aligned, and fastened in position with new concrete foundations. Forms are to be placed and secured, post-tensioning cables installed, and concrete placed, finished, and cured. After the forms have been stripped and the concrete properly cured, the post-tensioning cables will be addressed as required. Proposals must be submitted as directed, with all forms fully completed and executed, in ink or typewritten, and include all required signatures and notarizations. Proposals must be single-sided, bound or stapled, and proposers must make all requisite copies of required forms. The proposer is solely responsible for ensuring the proposal is received at the specified location by the deadline. Proposals must include: Contract Compliance Documents; Addendum/Certification/Notice of Acceptance and Non-Collusion Statement; Price Proposal; Proposal Security (as required); Contractor's Qualification Statement; and the CHRO Contract Compliance Monitoring Report. The proposer must fully complete all compliance documents, including the Certification Regarding Debarment, Annual Statement of Financial Interests, and Disclosure & Certification Affidavit Regarding Outstanding Obligations.
