The project is for the Repair and Replacement of Interior Finishes at Helena – West Helena, Arkansas. Bids must be submitted in a sealed envelope plainly marked with the address of the bidder, job number, and date and time of bid opening. Bidders are required to have an Arkansas Contractor’s License. Each bid must include a completed and signed Representations, Certifications, and Other Statements of Bidders (form HUD-5369-A), a signed Bid Form, a Bid Guarantee equal to five percent of the bid (which can be a verified check, cashier’s check, U.S. Government Bond, or bid bond), and a completed, signed, and notarized Non-Collusive Affidavit. Additional required forms include Certification of Bidder Regarding Section 3 and Segregated Facilities, Contractor Section 3 Plan Format, Previous Participation Certification (HUD-2530), Certification of Prime Bidder Regarding Equal Employment Opportunity, Certification of Prime Bidder Regarding Non-Segregated Facilities, and Contractor’s Certification Concerning Labor Standards and Prevailing Wage Requirements. The successful bidder must furnish a Performance and Payment Bond or Bonds in the amount of 100 percent of the Contract Bid for contracts exceeding $25,000. Failure to include any of the required, completed documents may render the bid non-responsive. The Public Housing Agency reserves the right to reject any and all bids or to waive any informality in the bidding. No bidder may withdraw their bid for thirty days after the bid opening without the consent of the Public Housing Agency.
