Bidders must provide a complete response to the items indicated in the RFP Structure and Documentation table. References and links to websites or external sources may not be used in place of the required information within the proposal. The bidder's company name should be included in the header of all documents submitted. All documents and information submitted become public record upon receipt. The required documents are: Confidential Treatment Form, Vendor Questions Worksheet, Schedule A - Statement of Work, Schedule B - Pricing, Attachment 1 - Contractor Qualifications, and Attachment 2 - Contractor Experience and Portfolio Examples. Mandatory documents must be completed and attached with the proposal in SIGMA VSS. The proposal and all attachments must be fully uploaded and submitted electronically prior to the proposal deadline. Proposals may be posted on the State's publicly available website after bidders are notified of the award recommendation. Contact with any other State personnel, agent, consultant, or representative regarding this RFP may result in disqualification. If modifications to the RFP are necessary, they will be posted on the specified website. Questions must be submitted in writing to the Solicitation Manager only and responses will be posted publicly. File sizes for attachments are limited to 30 MB per document. Failure to submit as required may lead to disqualification.
