Contact information Name Jacob Wolgamood Address: 229 South Second Street Elkhart, Indiana 46516 Phone: 574-522-4855 x3132 E-mail: jacob.wolgamood@cityofelkhartin.gov South Main Street Streetscape Improvements Phase 4 (from Freight Street to Prairie Street): Includes asphalt roadway reconstruction, asphalt milling and resurfacing, removal and replacement of concrete curbs, sidewalks, and approaches, installation of new ADA ramps and pedestrian crossings, removal and replacement of lighting, water main and services, sanitary sewer and laterals, installation of new stormwater sewer, and tree planting on South Main Street between Freight Street and Prairie Street. Bids must be submitted on the State of Indiana Contractor's Bid for Public Work - Form 96 (Rev. 2013). Each bid must include a certified check or bank draft drawn on a solvent Indiana bank, payable without condition to the City of Elkhart, or a satisfactory bid bond executed by the bidder and surety company in an amount not less than five percent of the bid. Any deviation from the bid documents must be explained by the bidder. Bids, proposals, or quotations submitted by a trust must identify each beneficiary of the trust and each settler empowered to revoke or modify the trust. The contract will be awarded to the lowest responsive and responsible bidder. The Commission reserves the right to waive informalities or irregularities and to reject any and all bids or portions thereof.
