The work includes all labor, material, equipment, and supervision required for preventative maintenance and rehabilitation of bridges, as specified for each structure. This includes surface preparation and construction of bridge deck overlays (latex modified portland cement concrete, latex modified concrete very early strength, or silica fume modified concrete), patching of concrete overlays, removal and construction of portions of concrete curb, placement of revetment riprap, installation of geotextiles, seeding and sodding, cleaning and epoxy coating of existing piles, and surface sealing of designated bridge elements. Specific pay items cover the work, such as "Patching Concrete Structures" (SFT), "Bridge Deck Patching, Partial Depth" (SFT), and "Warranted Polymer Overlay" (with performance warranty as specified for bridge decks). Additional requirements include coordination for road restrictions, disposal of excess material, and all necessary testing and certification of materials by approved independent laboratories. All work must conform to applicable Indiana Department of Transportation Standard Specifications and referenced sections. Only prequalified contractors for specified bridge work categories may bid, and each bid must be accompanied by a certified check or Bidder's Bond equal to at least 10% of the bid amount. Required documents include Form 96, Non-Collusion Affidavit, Employment Eligibility Verification Certification, Drug Testing Program Compliance, and an itemized proposal. Bids must be submitted electronically through the county’s eProcurement portal, and all materials and labor must meet the requirements detailed in the provided drawings and specifications. The Owner reserves the right to waive informalities and bids are valid for 90 days.
