This work includes, but is not limited to, the installation of a 36" Butterfly Valve south of the intersection of Lake Ave. and E 120th St. on the west side of the road, and south of the intersection of Lake Ave. and E 124th St. just to the west of the George Lake Trail. The work also includes the installation of two 36" line stops at each location, a total of four, including excavation and restoration. All work is located in the City of Hammond, Lake County, Indiana. All bidders must be properly licensed in the City of Hammond at the time bids are received. Bidders must comply with local ordinances related to responsible bidding practices and must be prequalified by the City of Hammond Building Commissioner prior to submitting a bid. Bids must be properly and completely executed on the provided bid forms. Each bid must be accompanied by a certified check, cashier's check, or bid bond in an amount not less than five percent of the total bid price. A performance bond and payment bond, each in the amount of one hundred percent of the contract price, will be required. Standard Indiana Form 96 (Revised 2013) including non-collusion affidavit, and an Affirmative Action Statement must be submitted at the time of bid. Contractors must subscribe to the Indiana Plan and a joint apprenticeship and training program if their bid is accepted and provide proof before any payments are made.
