The City of Sierra Vista is soliciting proposals from qualified companies to provide veterinary services under an annual contract. The successful proposer must have been in the business of providing veterinary services as described for at least three years. The scope of work includes three options: (1) providing veterinary services at the proposer’s business where Sierra Vista animal control staff will deliver animals for as-needed services prior to adoption; (2) providing part-time veterinary surgical services at the City's surgical center at the Nancy J. Brua Animal Care Center; and (3) submitting alternate proposals for services not specifically requested. Proposers may submit responses for any or all of these options. A one-year contract will be awarded, with the possibility of up to four annual extensions, with contracts potentially awarded to more than one provider. Bidders are required to obtain and maintain specified insurance coverage throughout the contract. Proposals must adhere to all prescribed requirements and formats, including completion of all required forms and signatures. Unsigned or incomplete proposals will be rejected. Proposers must include anticipated costs, and all documents must be submitted electronically via the City's eProcurement portal. Proposals must include proof of insurance, a signed non-collusion statement, transaction privilege tax information, and appropriate tax forms. Proposals are valid for 120 days from signature date. Deviations from requirements must be clearly stated and approved in writing. The City may reject any or all proposals and reserves the right to cancel the solicitation or award, re-issue the RFP, and select the highest ranked responsive proposal based on evaluation criteria such as licenses, experience, available services, pricing, and location. Costs for preparation of proposals are the sole responsibility of the proposer and will not be reimbursed.
