Furnish all labor, materials, equipment, and supervision necessary for the construction of an accessible playground, asphalt basketball court, parking area, and associated site improvements. Work includes, but is not limited to: erosion and sediment control; demolition and removal of existing pavement, culverts, and site features; installation of a playground system with fall protection surfacing; asphalt and concrete pavements; ADA-compliant parking and access; drainage improvements; landscaping; and miscellaneous site amenities. Each proposal must be accompanied by a deposit or Bid Bond in the amount of 5% of the bid. The apparent low bidder will be required to submit a completed New York State Vendor Responsibility Questionnaire For-Profit Construction (CCA-2) and M/WBE-EEO and SDVOB Utilization Plans within 24 hours and within five days, respectively, of being notified that they are the apparent low bidder. Subcontractors will also be required to complete the Vendor Responsibility Questionnaire. There are maximum subcontracting limits as specified in the Supplemental Instructions to Bidders. All contractor employees must be certified as having completed an OSHA 10 safety training course. Successful bidders may be required to furnish a performance surety bond and a labor and materials surety bond each for 100% of the contract amount. All successful bidders will be required to provide policies of insurance as set forth in the contract documents. Proposals will be accepted only from bidders who obtain the contract documents directly from the Department and must be submitted on the official Proposal Form enclosed in a sealed envelope. Specification Section 007383 XIX (b)(1) “Contract Goals” is revised to change the overall Minority and Women-Owned Business Enterprise (MWBE) participation goal from 0% to 30%.
