The City of Toledo DPS/Facility of Operations is requesting bids for lighting replacement and upgrade for Believe Center to LED Type of Award Lump Sum MBE - Minority-Owned Business Enterprise WBE - Woman-Owned Business Enterprise Bid Valid 180 days Start/Delivery Date Upon Issued PO Project Duration 6 months Question Deadline 05/07/2026 at 4:00 PM ET Online Q&A Yes. Questions must be submitted thru electronically. Annual Paperwork: W-9 Insurance Certificate, naming the City of Toledo as an additional insurer - requirements available online EEO Compliance Debarment Certification Living Wage Affidavit, updated no later than February 1st Bureau of Worker's Compensation Certificate As noted above, City forms must be filled out and attached to the bid. Project Paperwork includes the following: Acknowledgment and Acceptance of Conditions Contract Specifications, Primary and Subcontractor Forms, Living Wage Contract Addendum Other required forms include those noted in the Response Type Section and also noted in the Other Details Section: Signed Contract, Bid Guaranty 2.5%. The following items will be required upon awarding the contract: Payment Bond, Performance Bond and PLA, if applicable. The successful bidder shall submit certified payrolls to the City for all labor performed on the project 2 weeks after start date then weekly thereafter.
