Amendment 0001 deletes the requirement to furnish, install, and commission the Mojave Monitor dry vacuum system or like model from the Statement of Work, B.1 Deliverables. This combined synopsis/solicitation is issued under FAR Part 12 for commercial products or commercial services and constitutes the only solicitation; a separate written solicitation will not be issued. Solicitation number 36C26026Q0595 is issued as a request for quotation for the acquisition of annual backflow assembly testing, maintenance, and compliance certification services. The acquisition is set aside for small business concerns and incorporates provisions and clauses by reference, with full text available electronically at acquisition.gov. The scope of work includes annual testing, maintenance, and compliance verification of 93 backflow prevention assemblies. Vendors must be registered in the System for Award Management (SAM.gov) and must be certified by the Oregon Health Authority. Location: VA Southern Oregon Rehabilitation Center & Clinics, 8495 Crater Lake Highway, White City, OR 97503.
