Renovation of a municipal facility in Thiells, New York. Completed plans call for the renovation of a municipal facility.

Building 3 Exterior Door Replacement. Be sure to include a cost breakdown (materials, labor, and overhead) and list number. Contractor is to provide and supply a 1-year warranty on material and labor. Mandatory scheduled site visit required; attendance must be confirmed and recorded on the project sign-in sheet to be eligible to submit a quote. Quotes must include a breakdown of materials, labor, and overhead, and be completely filled out on the provided quote proposal form. Submission of quotes must be made by the specified deadline and must include correct prevailing wage rates. Vendors must demonstrate 2 years of relevant experience upon request. No subcontracting is allowed. Award is made to the most responsible and responsive vendor providing the lowest cost meeting all requirements. The awarded vendor must submit certificates of Liability Insurance, Workers Compensation Insurance, completed Lobbying Law Forms, and signed Appendix A. All contracted employees must be paid prevailing wages as required by NYS Labor Law, and certified payroll records must be submitted with invoicing. Bidders must provide their NYSDOL registration number and Certificate of Registration at the time of bid submission or be disqualified.
