Site work and paving for a sidewalk / parking lot in Larchmont, New York. Completed plans call for site work for a sidewalk / parking lot.
Mandatory site visit is required. When submitting a quote, include a cost breakdown for materials, labor, and overhead, and list number. Submit within 30 days of receiving the purchase order. Contractor must provide and supply a 1-year warranty on material and labor. Attendance at the site visit must be confirmed and recorded; late arrivals will not be allowed to participate or submit a quote. Quotes must be received by the stated deadline and include prevailing wages in accordance with the wage schedule. Vendor must have at least 2 years of relevant experience and provide certificates of Liability Insurance and Worker’s Compensation Insurance, completed Lobbying Law Forms, and a signed Appendix A. Subcontracting is not permitted. Upon award, provide a list of materials; HVDDSO will purchase item #4 Stone and asphalt. The vendor is responsible for trucking materials, proper installation, and returning scale house tickets. Vendor must submit certified payroll records with invoices. Quotes must be submitted using the provided quote proposal form; other forms will not be accepted.
