Please provide a copy of your firm's certification documents when submitting a quote. OPWDD is soliciting quotes from responsible and qualified MWBE, SDVOB or NYS small business contractors to provide front deck/ramp replacement service to Sunmount DDSO, SOIRA 134 Leroy St. Potsdam, NY 13676. All questions or requests for information shall be submitted, in writing to kyle.m.beaudette@opwdd.ny.gov. Deck replacement. The vendor shall provide all labor and materials necessary to complete the work in a professional manner according to scope of work and architectural drawings if supplied. Work is to be completed within 30 days of award, unless modified below. This is a NYS Public Works project, prevailing wages are to be paid and Certified Payrolls must be submitted at time of completion. Refer to attached specification for the following scope of work. Site must be cleaned, and all materials secured at the end of each workday. Absolutely no scraps, especially fasteners, are to be left unsecured. Upon completion of the deck replacement, before removal of equipment and scaffolding, contractor is to schedule a quality control and quality assurance inspection with Plant Superintendent or his/her representative. SCOPE OF WORK includes, but is not necessarily limited to the following: 1. Demolition of existing Front of home Deck/Ramp, including footings, and properly dispose of materials. 2. Excavation and installation of new Deck/Ramp footings meeting the requirements of the Building Code of NYS. Vendor shall be responsible for contacting local utilities (Digsafe) prior to excavation to ensure any underground utilities are clearly marked and protected from damage. 3. New Deck/Ramp shall be ADA compliant per architectural drawings. 4. Framing of deck structure (including stairs). All framing shall comply with the Building Code of NYS. 5. Installation of decking material. Decking material is to be in accordance of design 1"x6" composite deck board installed per manufacturer's instructions. Colors to be chosen by OPWDD staff. 6. Installation of composite deck rails in accordance with Design. Railing installation shall comply with the Building Code of NYS. 7. Restore all affected lawn, driveway or property damage incurred during the project to conditions prior to start of project.
