If proposing an alternate brand, provide the alternate brand name, catalog/part number, and characteristics equivalent to the referenced brand. Also, submit documents clearly describing the alternate for evaluation. If offering equivalent equipment, vendors must provide, at no cost if asked: supporting documentation, software if needed, transportation, set-up, removal, and equipment for examination or demonstration. Successful contractor must submit valid and current NYS Workman’s Compensation and Disability Forms and complete documents related to NYS Finance Law §139 j&k, and Public Officers Law. Quotes must be received by the stated due date.
