The work under this Contract includes construction of approximately 880 linear feet of 8 water main including service relays, and water construction staking; 570 linear feet of 8 sanitary sewer main construction, 300 linear feet of 30 sanitary sewer CIPP lining, sanitary sewer lateral reconnections, 1050 linear feet of pipe bursting sanitary sewer laterals, 570 linear feet of 12 storm sewer main construction, including storm sewer lateral construction, inlet leads, storm sewer inlets; 1860 cubic yards of excavation, 1820 linear feet of curb and gutter, 500 cubic yards of 1 1/4 crushed aggregate base course, 490 cubic yards of breaker run, 500 tons of HMA pavement, and 8300 square feet of concrete sidewalk and driveway reconstruction; construction staking, and restoration. All bidders shall provide proof of responsibility on the form furnished by the Director of Public Works and it shall be filed with the Director of Public Works not less than five (5) days prior to the time set for opening of bids. Said proof of responsibility shall not be valid if filed prior to one year of the date of opening bids. All proposals must be submitted through QuestCDN.com via their electronic vBid online bid service provided for that purpose and issued to the specific bidder by the Director of Public Works together with a bid bond equal to at least five (5) but not more than ten (10) percent of the bid payable to the City of Green Bay as a guarantee that if the bid is accepted, the contractor will execute and file the contract and a performance bond in the amount of one hundred percent (100%) of the total bid within ten (10) days after the award of the contract.
