The project consists of the alteration of the customer service desk, the creation of a security office, and an inspections and planning office, as well as a new family toilet room, lactation room, touch down office, and storage rooms. Separation of public space from private city offices will be created with a new cross-corridor door. Bidders must fill out and include the following forms with their bid proposal: Bid Forms, Contractor Qualifications, Non-Collusion Affidavit, Bidder Safety Evaluation Questionnaire/Contractor Safety Record Information, and Equal Business Opportunity Program Construction Forms (including UBE Participation on Base Bid and on Bid Alternates). Any bid submitted without these forms may be deemed non-responsive. Bidders are responsible for submitting the required Underutilized Business Enterprise (UBE) Construction forms to meet project goals of MBE 11% and WBE 0%. Each bidder must show evidence of being licensed under Chapter 87 of the N.C. General Statutes with a General Contractors License. All inquiries, questions, requests for substitution, and/or clarifications must be submitted in writing according to the given directions before the specified deadline. The City of Durham reserves the right to reject any or all bids, waive informalities, and/or accept such bid as appears to be in its best interest.
