Work includes the rehabilitation of W. Wisconsin Avenue in the Village of Pewaukee. The scope involves approximately: removing 210 LF of concrete curb and gutter, 1,310 SF of concrete sidewalk, 110 SF of concrete driveway, 545 CY of excavation below base course, pulverizing 7,215 SY of asphaltic pavement, 265 LF of pavement sawcutting, installation of 210 LF of 30” concrete curb and gutter, 1,310 SF of 5-inch concrete sidewalk, 45 SF of curb ramp with detectable warning field (yellow), 110 SF of 7-inch concrete driveway, 1,245 TON of base aggregate dense, 2,275 TON HMA pavement, restoration (4” topsoil and hydroseed) as a lump sum, 1 LS of traffic control, 6,080 LF of epoxy marking line, 10 water valve box replacements, 21 inlet protections, 29 storm structure repair adjustments, 7 sanitary manhole adjustments, and 49 VF of sanitary manhole epoxy lining. This is a unit price contract. The schedule of estimated quantities is for determining the low bidder. Bidders must complete all required blanks in the electronic bid form and submit bid security of 5 percent of the maximum bid price. Bids without all required subcontractor and supplier information may be rejected. Successful bidders must provide performance and payment bonds in the full contract amount and list all subcontractors and suppliers with values over $1,000.
