Amendment 4 posted regarding to Governmen's responses to questions and attached revised Statement of Work (SOW) The purpose of this amendment is to provide the Government's responses to questions received, incorporate the revised Statement of Work (SOW)into this solicitation. A. See attached revised Statement of Work (SOW). B. See attachment for the Government's responses to Questions received. C. No other changes to the terms of this solicitation. NOTE: Offerors must sign/acknowledge all posted amendments.__ Remove, legally dispose of, and replace existing degraded steel manhole covers associated with the petroleum Underground Storage Tank (UST) infrastructure. The work includes providing all labor, materials, equipment, and supervision necessary to perform a one-time replacement program intended to improve personnel safety, enhance corrosion resistance, eliminate electrical conductivity risks, and ensure long-term structural durability. The contractor shall perform the required cleaning and remediation activities in accordance with the Statement of Work (SOW) and deliver a complete and compliant replacement of the affected manhole cover infrastructure. Period of Performance: 08/11/2026 to 09/10/2026. All Questions are due by 07/30/2026 at 5PM, to the Contracting Officer via e-mail to ravilusd@usmma.edu. SCOPE OF WORK The Contractor shall furnish all labor, materials, equipment, transportation, tools, disposal fees, and supervision necessary to perform the following tasks: 4.1 Demolition and Disposal o o Large Covers : Remove and legally dispose of five (5) existing standard-size steel UST manhole covers (approximately 36 to 42 inches in diameter). Small Covers: Remove and legally dispose of five (5) existing small-size steel UST fill port manhole covers (approximately 12 to 16 inches in diameter). 4.2 Procurement and Installation . Large Covers : Furnish and install five (5) standard-size (36 to 42 inches) fiberglassreinforced polymer (FRP) manhole covers. Small Covers: Furnish and install five (5) small-size (12 to 16 inches) fiberglassreinforced polymer (FRP) fill port manhole covers. 4.3 Field Verification & Engineering Execution Field Verification: Validate all exact physical field dimensions prior to material procurement, fabrication, or delivery. Fitment & Alignment: Ensure all new covers achieve proper seating, lock into place seamlessly, match finished grade alignment, and eliminate tripping hazards. Subcontractor Management: Maintain full accountability for the performance, safety compliance, and quality control of all utilized subcontractors. Period of Performance: 08/18/2026 to 09/17/2026
