Medicaid/400 Hall Shower Renovation Demo and remove all existing floor and wall tiles. o Demo and remove exiting plumbing fixtures, mirror, electrical outlets, lights, grab bars and paper holders. o Install ceramic on the walls in the same way it exists. Wainscot should stop below the light switch or GFI at the sink, whichever is the lowest. Cap the tile of the wainscot with a smooth strip in a nickel finish. Remove and replace all wall board in the shower. Install (Kerdi-Material waterproofing membrane) in all areas where ceramic tile is to be installed. Run the membrane from the floor slab up the walls in all areas. Corners also need waterproof membrane. Provide materials, equipment and labor to install new wall and floor tiles in current configurations. (Daltile - Articulo - Beige Rectangle - Color AR07, 12 x 24 walls, 1 x 3 floors. Grout: Prism #09 Natural Gray). o Prep and paint wall areas where no tile is installed - one coat primer and two satin finish coats (SW Useful Gray 7050 Satin finish).). o Prep and paint door and door trim (SW Alabaster 7008 - gloss finish) o Prep and paint ceiling - (One coat primer and Two coats eggshell White). - Provide and install (2) (1x4 low profile LED lights, white - Lithonia). Provide and install (2) (LED can lights). o Provide and install (1 ) (white ADA toilet with white split seat). Provide and install new (braided water lines, 1/4 turn shut off valves, new waste lines and ADA pipe wraps) where applicable. o Provide and install new wall mounted sink Provide and install new wall mounted sink (Kohler Greenwich -model#2032-0 -White). Provide and install new sink faucet (Delta 2529LF-HDF Polished Chrome). o Provide and install (1 ) new Delta single handle shower valve body and trim o o (T13291LHD & R-10000-UNBX, polished chrome). Provide and install (1 ) new shower head and flexible hose (Moen - Attract - Chrome - 6 function 5 1/2" dia. Spray head handheld shower - model#26603). Provide and install (1 ) (24" x 36" stainless steel framed mirror). o Provide and install new (hospital grade) electrical GFI outlets and switches and appropriate (gray) cover plates. o Provide and install new brushed nickel grab bars (match existing sizes and locations. Add 2x10 yellow pine backer boards as needed. Provide and install (2) (GBS white Phenolic fold down shower seats with x 16"). Add 2 x 10 yellow pine backer boards.Cottage/100 and Medicaid/400 Hall Resident Room TV Outlets Add new hospital grade duplex electrical outlets and coax TV box's with cable connection. (2 per room) Reroute the cable wires to the new coax boxes. o o Run new wiring to new electrical outlets. o Facility will remove existing T.V's and T.V. brackets. Cottage/100 Hall Resident Room Renovations (17) Rooms- o Remove and dispose of all existing flooring and base. Perform moisture and Alkaline testing on concrete floors prior to LVP installation. o Prep floor as needed. o Use manufacturers recommended concrete floor sealer and moisture resistant adhesive. o Provide and install new LVT flooring and base (Shaw Abide 4107V -Conch Shell 07140) (Johnsonite 4" traditional vinyl cove base - Burnt Umber B #63). o Provide and install (17) ADA white toilets with white split seats. o Provide and install (17) new wall mounted ADA sinks and faucets (Goodtime Medical - ADA Vanity 30"- SKU#A30-OS-sink base color: Absolute Acajou - Countertop Color: Sugar Pie), Faucet - (Order faucet that is available on the website for this sink unit). o Provide and install new braided water lines, new waste drains and 1/4 turn shut off valves. o Provide and install new brushed nickel grab bars.
