Project Control No. 1902812 Federal/State No. 1902812 Bridge Replacement 64-01009 Prestressed Concrete Bulb-t Beam Porter County - on Lenburg Road Over Salt Creek, 0.6 Mile West of Sr 149 Qualifications: D(A) 0250 Contractors desiring to submit electronic bids via Electronically must register with INDOT as a valid bidder by submitting the form provided on the INDOT Letting Information internet page. Failure to register will cause failure of the Electronic bid submission process. A bidder may request valid bidder status by submitting the bidder registration form until 9:00 AM on the bid opening date. An executed bid bond shall be included with all bids. Only electronic bid bonds submitted through the designated electronic medium, either Surety2000 or Tinubu Surety Solution, will be accepted with the bid. A fully executed performance bond shall be included with all bids, except on contracts for roadside mowing, vegetation control, demolition, landscaping, fencing, seeding and sodding, bridge painting, and guardrail installations of $100,000 or less, when the bidder may elect not to furnish a performance bond. Electronically does not support submittal of the performance bond. The performance bond must be submitted to the Construction Letting email inbox or be delivered on paper by the time set for receipt of proposal bids. Each contractor that submits a proposal bid in the amount of $150,000 or more for highway improvements described in this notice is required to submit with that bid a written plan for a program to test the contractor's employees for drugs. The plan shall be in compliance with Indiana Code 4-13-18. The plan must be submitted to the Construction Letting email inbox or deposited with the INDOT Prequalification Engineer by the time set for receipt of proposal bids. INDOT - Call 580 - Bridge Replacement - Lenburg Road Discharge fill material into two emergent wetlands totaling 0.028 acres to facilitate the replacement of Porter County Bridge 1009. The replacement structure will be a 114-ft long, simple span, prestressed composite bulb-tee beam bridge. This work shall consist of protecting migratory bird species as required by the Migratory Bird Treaty Act, MBTA, in accordance with 105.03. Recycled foundry sand, RFS, consists of a mixture of residual materials used from ferrous or non-ferrous metal castings and natural sands. The Contractor shall have the option of incorporating RFS into applicable operations in accordance with 105.03. This work shall consist of: (a)covering soil slopes with a cohesive soil to establish vegetation; (b)use of soil reinforcement materials and blankets for mulching seed as shown on the plans and in accordance with these requirements; (c)use of water absorption chemicals to hold water and keep moisture available for seed germination; and (d)soil testing for pH, nutrient supply, and organic matter percentage. As shown on the plans, the existing approach guardrail in all four quadrants of the project shall be removed and salvaged. This item shall remain the property of Porter County. The existing guardrail shall be set aside and Martin Bobcek (219-465-3608) of the Porter County Highway Department shall be contacted. The Porter County Highway Maintenance Department will remove the existing guardrail from the site. The existing bridge railing shall be removed and disposed of by the Contractor and included in the pay item Removal of Present Structure. BF&S is developing plans for the replacement of the existing bridge structure carrying Lenburg Road over Salt Creek, located approximately 0.6 miles west of S.R. 149, in Section 8, Township 36 North, Range 6 West, Portage Township, Porter County, Indiana.
